Registering a New Student

Student Registration Documents Needed

  1. Birth Certificate (with raised seal)
     
  2. Four (4) Documents with Proof of Address dated within the last 60 days. * – Denotes financial information or license numbers can be blacked out.  Acceptable documents include:
    • Deed *
    • Lease/Rental agreement with signatures & date *
    • Mortgage Statement *
    • Settlement Statement (HUD statement from closing) *
    • Current tax bill from the Municipality *
    • Utility Bills (cable, electric, gas, water, etc. (NO cell phone bills) * 
    • Valid Driver’s License *
    • Automobile Registration
    • Affidavit of Residency (notarized in New Jersey)
  3. Immunization Records documented by a physician or health clinic
  4. Physical Examination Form (completed within the past 365 days & signed by a physician)


Other Documents (if applicable)

  • Latest report card from former school (copy)
  • Latest transcript/test scores from former school (copy)
  • Request for Release of Records Form – signed & dated (Transfer students)
  • Custody or Guardianship Papers
  • Child Study Team records
  • Student Athlete Residency Affidavit (students in grades 9-12 who wish to participate in athletics)


Frequently Asked Questions

  • What happens if my name is not on the lease/rental agreement, deed, mortgage statement or municipal tax bill?

An Affidavit of Residency form must be submitted. The property owner/renter AND the parent/guardian must complete and sign the form and have it notarized in New Jersey. See this Affidavit of Residency Form

  • What happens once all of the documents are submitted?

Once all of the required documents are submitted with the online registration form, the registrar will review them to determine if more information is needed.  A school will be assigned to your student.  You will receive an email confirmation that your child’s registration has been finalized.  A school staff member will contact you with more information.

  • How long does the registration process take?

After all of the required documents are submitted with the online registration form, the review process can take up to 3-business day.  You will receive a confirmation email from the Registration Department when your child’s registration has been finalized.
